Thank you for your interest in CIS.  Before activating your account we need to obtain the following information: 

1.  A fully completed and signed CIS Contract and Credit Score Addendum.  
2.  Copy of your valid Mortgage Broker license.
3.  Current business card or company letterhead   stationery with valid business phone number.
4.  A sample borrower's signature authorization form.
5.  Copy of current business lease. 
6.  Copy of principal's drivers license.

Once CIS receives your completed application we will then move forward to obtain the following:

1. A physical inspection of your place of business which we're required to outsource to a third party.
2.  A credit report for the principal of your company. This report will not affect the individual's credit score.
